Submit the Leave of Absence/University Withdrawal Form through the Google Form HERE.
*LoA/Withdrawal form is attached to this page.
If you need academic advising, you must arrange a meeting with your department coordinator.
The Leave/University Withdrawal process takes 1-2 business days. If you do not receive an email, it means your leave/university withdrawal has been successfully processed. We will contact you if any revisions to the forms are required.
*Any incomplete form may result in rejection and a delay in the process. Please ensure that you submit the forms with accurate information by following the sample below.*

All the information should be filled out in English.
Section II: Select the term for which you are applying for the leave/withdrawal.
Section III: Select 'Leave of Absence' or ‘University Withdrawal’
*SUNY Korea students going on military leave should still mark “Leave of Absence” on the form.
A student signature is required.
Date: Students must fill out the date on which they submit this form. Any discrepancy between the written date and the actual submission date will result in the form being rejected and requiring revision.
Students who need to extend their leave beyond four semesters must have an advising sessionwith their department and obtain approval for the Leave Extension Petition Form.
Students who take a leave of absence for two or more semesters will be assigned a new requirement term and will be responsible for meeting the new curriculum in effect at the time of their return. (SBU Bulletin)
Requirement Term Exemption for Military Leave: The exemption to revert the requirement term is available only to students who immediately take military leave and return directly to the term following their discharge. To verify eligibility for the exemption, students must submit their Military Document (병적증명서) within 2 weeks of their discharge date.
Your NetID & Stony Brook email accounts will be deactivated after 200 days of leave. If contact is needed, emails will be sent to the student’s registered personal email address.
Students are responsible for checking the tuition liability in the Academic Calendar and submitting the required documents to process the tuition refund. More information is available on the SUNY Korea website.
